Research Objectives: To investigate the relationship between the protein-bound uremic toxins, P-cresol sulfate and Indoxyl sulfate, and estimated Glomerular filtration rate (eGFR) and to determine the relevance of renal deterioration and cognitive Function in Individual with Chronic Kidney Disease.

Design: In this study, we measured the level of cognition-related uremic toxins, estimated glomerular filtration rate, and took advantage of a non Paper-and-Pencil Test: Cambridge Neuropsychological Test Automated Battery (CANTAB) to cognitive changes in patients suffering from CKD.

Setting: This study execute at a tertiary hospital in Taipei with informed consent.

Participants: Thirteen participants enrolled in our study and signed a informed consent.

Interventions: No intervention
R was adopted for statistical analysis to evaluate the relationships between variables.

Main Outcome Measures: The subjects took three subtests of CANTAB test, including Rapid Visual Information Processing (RVP), Spatial Working Memory (SWM), and Delayed Matching to Sample (DMS). Blood tests about renal functions were also received.

Results: For Protein-bound uremic toxins, there were positive correlations between P-cresol sulfate and estimated Glomerular filtration rate (eGFR). The same trend was also noticed about Indoxyl sulfate. In cognitive tests, for those who had better renal functions, the following standard scores of subtests showed higher: RVP-A prime (A’), SWM-Strategy, DMS-percentage of correction in all delays section, and DMS-Probability of Error Given Error.

Conclusions: We found that uremic toxins accumulated as the renal function declined, and three portions of cognitions: executive function, working memory, and attention and psychomotor speed all showed decreased as renal function deteriorated. However, the results of this study should be validated in future studies with larger sample sizes or longer period of clinical trials.
