Research Objectives: Summarize the post-discharge return to school recommendations, for individualized education plans (IEP) or 504 plans, for patients (N = 44) with traumatic brain injuries (TBIs) 2021 – 2023 from a pediatric rehabilitation unit in a tertiary care center via a hospital-based school program (HBSP).

Design: Retrospective chart review.

Setting: Inpatient.

Participants: Pediatric patients with TBI admitted to inpatient rehabilitation.

Interventions: Not applicable.

Main Outcome Measures: Sex, race/ethnicity, age, and length of hospital admission.

Results: Across 44 patients with TBIs receiving HBSP services on the pediatric rehabilitation unit, 5 were recommended IEP only, average age was 9.40 years (SD 4.39), 4 were females and 1 male, all identifying as White, non-Hispanic/Latino (N = 5), and average length of admission (LOS) was 51.00 days (SD 23.42).

504 only recommendations were made for 17, average age was 14.00 years (SD 4.39), 11 were males and 6 females, identifying as Black (N=5), White, non-Hispanic/Latino (N=11), or “Other” (N=1), and average LOS was 18.29 days (SD 10.88).

Both IEP and 504 recommendations were made for 5, average age of 12.00 years (SD 4.53), 3 males and 2 females, identifying as Black (N=3) or White, non-Hispanic/Latino (N=2), and average LOS was 27.40 days (SD 15.01).

No recommendations were made for 17, average age of 10.41 years (SD 3.84), 12 males and 5 females, identifying as Asian (N =1), Black (N = 4), or White, non-Hispanic/Latino (N = 12), and average LOS was 10.65 days (SD 14.00).

Conclusions: In this program, most patients received IEP, 504, or both recommendations as part of return to school planning. No distinct gender or race/ethnicity differences occurred across groups. Patient age ranged from 10-14 years. LOS differed across recommendation types, with patients receiving IEP only (51.00 days), IEP and 504 plan (27.40 days), and 504 plan only (18.19 days) recommendations having longer admissions compared to those who received no recommendations (LOS 10.65 days).

Learning Objectives:

  • Experiencing a traumatic brain injury often interferes with adolescents ability to engage in school, resulting in significant disruptions in their education. It is important for the healthcare team to recognize and attempt to mitigate these educational disruptions
  • Hospital Based School Programs (HBSPs) may help minimize education disruptions by providing an opportunity for education professionals to work with adolescents during their hospitalization to identify whether they would benefit from additional resources and communicate their recommendations to the adolescent's school program.
  • Length of hospital stay following inpatient treatment for traumatic brain injury can impact adolescent's educational needs, including need for 504 plans and individualized education programs.
