Research Objectives: To evaluate the effectiveness of a 16 week virtual at-home therapy program that integrates Occupational Therapy and Rehabilitation Technology for patients post stroke.

Design: Case Study

Setting: At-home therapy with an Occupational Therapist through a virtual platform.

Participants: A 43-year-old male, who experienced a stroke in May 2003 following a motor vehicle accident, resulting in right-side hemiparesis. He had limited use of his right upper extremity. Goals were to regain function and enhance his quality of life, particularly for cooking and fishing.

Interventions: After a comprehensive evaluation, the patient participated in a technology onboarding session using the NeuroBall Platform and weekly one-on-one virtual therapy sessions. Each week, the patient was provided with a personalized therapy plan that included using the Platform, therapeutic exercises, and ADL task-specific training activities. The therapist remotely monitored the patient for adherence and communicated with the patient weekly regarding progress.

Main Outcome Measures: Patient adherence, Modified Rankin Scale, VAS Pain and Fatigue Scales, EQ5D, Overall Health Score, Functional Upper Extremity Level (FUEL), Range of motion, Stroke impact scale- hand function, Motor Activity Log (MAL) for amount of use (AOU) and quality of movement (QOM).

Results: Significant improvements were observed in upper limb function for active and passive range of motion, improved Stroke Impact Scale-Hand function (from 8 to 15), improved MAL scores for AOU (0.64 to 2.57) and QOM (0.71 to 2.43) and improved FUEL level from Gross Assist to Semi-Functional.

Conclusions: The virtual at-home rehabilitation program improved upper limb functionality and daily living activities in a chronic stroke survivor, demonstrating the potential for significant functional gains even 20+ years post-stroke.
