Identification of people with Alzheimer’s disease and related dementias (ADRD) is important for demonstrating value and for advancing and improving care for this population. This symposium will focus on how to build a framework for identification of persons living with ADRD using the electronic health record, review the characteristics and services used to predict readmission and include a discussion of the opportunities, challenges, and strategies for prevention of readmissions after discharge from the acute care hospital for the ADRD population.

Learning Objectives:

• identify persons living with dementia at point-of-care hospitalization using the electronic health record
• predict 30-day readmissions utilizing baseline patient characteristics and utilization of services
• identify how discrete clinical and operational data can be synthesized to create knowledge including opportunities, challenges, and strategies for prevention of readmissions
