As healthcare costs continue to rise there is an increasing need to provide high-value healthcare. Value is defined as the outcomes achieved per dollar spent, where the outcomes reflect aspects of health that are meaningful to patients. However, there are barriers to implementing patient centered outcome measures into clinical care. In September 2024, the Learning Health Systems Rehabilitation Research Network (LeaRRn) and the Center of Health Services Training and Research (CoHSTAR) will gather experts across the rehabilitation community for the Power of Patient Centered Outcomes in Rehabilitation Learning Health Systems summit to discuss barriers and opportunities related to integrating patient centered outcomes into clinical care. This symposium will disseminate the key highlights from this summit with a focus on ongoing challenges and potential solutions to implementing patient centered outcome measures in routine clinical practice.

Learning Objectives:

• Upon completion, participants will be able to discuss the importance of a multidisciplinary team in the implementation of patient centered outcome measures.
• Upon completion, participants will be able to describe ongoing barriers related to selecting, capturing, accessing, analyzing, and utilizing patient centered outcome measures in clinical care.
• Upon completion, participants will be able to describe potential solutions to selecting, capturing, accessing, analyzing, and utilizing patient centered outcome measures in clinical care.
• Upon completion, participants will be able to discuss the importance of a multidisciplinary team in the implementation of patient centered outcome measures.
