Stroke is the fifth leading cause of mortality in the United States. The prevalence of stroke is expected to increase by 20% by 2030 as compared to 2012 driven by the increase in our aging population. With 795,000 strokes every year and 9.8 million stroke survivors in the United States, stroke is a massive public health problem. We present the framework and preliminary findings of our multidisciplinary Patient Recovery and Caregiver Interventions for Stroke Excellence (PRECISE) program. PRECISE is centered on an interdisciplinary collaboration between neurology, neurosurgery, and rehabilitation that leverages innovation and technology to prevent fragmentation of care throughout the continuum of care for patients and caregivers following stroke. In this symposium, we will present the clinical infrastructure for deployment of this initiative across the care continuum, from the neuro ICU through discharge to community and the outpatient setting.
