Self-directed Approaches to Recovery After Stroke: Bridging the Transition Back into the Community 4523

Current stroke rehabilitation therapy services focused on remediation of impairment and functional limitations are not adequate to translating functional improvements to real-world and participation outcomes for individuals re-integrating back into the community. Self-directed training and self-management supports are approaches that promote patient autonomy and control over their care and management of the chronic impact of stroke with minimal support from rehabilitation professionals. In this symposium, we will present examples and evidence of innovative interventions utilizing these approaches with stroke survivors living in the community. In addition, we will discuss key challenges and strategies to promote shared decision-making.
