Concussions present a substantial public health challenge due to their high incidence in sports, accidents, and combat. Traditional diagnostic methods, reliant on subjective symptom reporting and clinical assessments, highlight the critical need for objective, reliable, and rapid diagnostic tools. This necessity stems from the complex pathophysiology of concussions, characterized by biomechanical forces causing neurochemical and metabolic disturbances in the brain, leading to temporary functional alterations. Immediate and accurate diagnosis is paramount to prevent further injury and facilitate effective management.

The presentation showcases advancements including blood-based biomarkers and reviews emerging technologies, like microRNA in saliva, pupillary biomarkers, and oculomotor tracking. Advanced neuroimaging techniques such as DTI, SWI, fMRI, and PET are discussed for their enhanced sensitivity in detecting subtle brain injuries. These innovations mark significant progress in understanding and managing concussions, offering hope for improved patient outcomes and a shift towards more precise, rapid diagnostic approaches in traumatic brain injury care.

Learning Objectives:

  • Describe the pathophysiology of concussions, including the biochemical and metabolic changes that occur in the brain following traumatic brain injury.
  • Identify and evaluate the role of key biomarkers (GFAP, UCH-L1, S100B, NF-L, P-tau) in the diagnosis of concussions, including their dynamics post-injury and their potential diagnostic value.
  • Explore emerging technologies and advanced neuroimaging techniques (i-STAT TBI test, microRNA, pupillary biomarkers, oculomotor tracking, DTI, SWI, fMRI, PET) for their applications in concussion management and their ability to enhance diagnostic accuracy beyond traditional methods.
