This panel discussion will provide an overview of the Social Determinants of Health Model and provide exemplars of rehabilitation programs across the national who are collecting data on social determinants of health to inform their practice and program development. Panelists include leaders in rural and urban settings who serve stroke patients across the continuum of care.
