Learn what a Home & Community Specialized Neurorehabilitation therapy model consists of and how these interdisciplinary team services are implemented. Discussion of medical neurorehabilitation level of care access criteria and how Home & Community clinical models can fill gaps in access to specialized neurorehabilitation. Analyze data from both the Mayo Portland Adaptability Inventory-4 and Supervision Rating Scale in demonstrating statistically significant and durable outcomes produced by this clinical service delivery model.
Learning Objectives:
Upon completion, the participant will be able to describe the clinical approach utilized in home and community based rehabilitation models and details of how interdisciplinary patient centered plans of care are created.
Upon completion the participant will be able to explain research recommendations for the use of home and community models for both pediatric and adult patients with neurological injuries
Upon completion the participant will be able to discuss how gaps in access to specialized neurorehabilitation affect overall patient outcomes.
Upon completion the participant will be able to describe how home and community programs are utilized by referring and funding partners
Upon completion the participant will be able to discern statistically significant functional changes in MPAI-4 and SRS rating scales and how this data can be used to advocate for additional models of rehabilitative service delivery for patients with neurological injuries.
