To create a list of actionable and prioritized recommendations to improve care transitions for individuals with hip fracture from the perspectives of persons with lived experience, care partners, healthcare providers, and decision-makers in Ontario, Canada.

Learning Objectives:

Upon completion, participants will be able to list actionable recommendations to improve transitions in care for adults with hip fracture.
Upon completion, participants will be able to describe the importance of person-centred care and care partner involvement in promoting positive care transitions.
Upon completion, participants will be able to identify the most important and highest priority areas to target for future quality improvement initiatives.
Upon completion, participants will be able to list actionable recommendations to improve transitions in care for adults with hip fracture.
