Neurobehavioral and psychological symptoms are highly prevalent in post-stroke recovery, and there remains a significant treatment gap for behavioral health symptoms. Persons with stroke can experience diverse neurobehavioral and psychological consequences that can interfere with the rehabilitation process and long-term recovery1,6,7. Increased evidence suggests that social determinants of health (SDOH) impact mental health outcomes10, and mental illness can impact SDOH.  Management of post-stroke recovery necessitates a multi-dimensional approach due to individual differences in stroke severity, complexities in socioeconomic and biopsychosocial presentation, and limited access to resources and support post-discharge. Kandu HealthTM is an app-based digital health and clinical support platform that assists with home and community integration.  A digital post-stroke navigation program that incorporates targeted behavioral health intervention and provides escalation pathways to address time-sensitive behavioral health needs can reduce psychological distress, empower patient safety through resource management, and result in an auspicious impact on stroke recovery and outcomes.
