This session will describe the results of a series of focus groups with people with brain injury, their family members, and community-based service providers aiming to explore community support needs and care transition needs to better support the health, well-being, and participation of people with chronic brain injury. We apply these findings to the chronic disease model for brain injury, and suggest specific recommendations for community support services, partnerships, social determinants of health needs, and individualized community-based rehabilitation services.

Learning Objectives:

Upon completion, participants will be able to identify relevant social determinants of health that should be considered when examining long-term community and social support needs of people with TBI.
Upon completion, participants will be able to describe TBI survivors'' and family members'' perceptions of the strengths and limitations of existing sources of community and social support.
Upon completion, participants will identify sources of community and social support that can be leveraged or built to better support people living in the community with TBI and their families.
Upon completion, participants will be able to identify relevant social determinants of health that should be considered when examining long-term community and social support needs of people with TBI.
