Cognitive rehabilitation is crucial in assisting individuals in enhancing their participation in daily activities after stroke. This symposium will report the progress of an information education page (I/EP) focusing on rehabilitation Strategies and SMART Goal Setting for people who experienced cognitive impairments after stroke. The symposium will provide an overview of the following: (1) cognitive impairments after stroke, (2) post stroke cognitive impairment and dementia, (3) cognitive rehabilitation strategies that can be done at home and with a therapist, and (4) the use of SMART Goal setting to maximize cognitive rehabilitation.
