Cancer treatment can accelerate physiologic aging and increase one’s risk for frailty and associated poor outcomes. Clinical practice guidelines now recommend routine assessment and coordinated intervention for frailty for older adults with cancer. Yet, strategies to address the functional domains of frailty in adults of all ages with cancer are understudied. To better understand and optimize the functional domains of frailty for adults with cancer receiving systemic treatment, we implemented and studied a GA frailty index-driven prospective surveillance model in community-based care. In this symposium, we provide an overview of the GA frailty index-driven prospective surveillance model, present the findings of our pilot feasibility study, and provide practical insights regarding the outpatient rehabilitation needs of this population. Attendees will gain valuable insights regarding the provision and benefits of an integrated care model for adults with cancer who are frail and pre-frail, building towards a proactive, high value health system.
